HEALTHCARE SURVEY (for MARRIED Malay/Chinese Female age 25yo – 40yo) ~ RM80 - RM100 (CLOSED)

We are looking for 70 respondents to participate in our survey.

Requirements:
- MARRIED Malay / Chinese Female
- Age 25yo – 40yo
- Working adults / Housewives
- Diarrhoea Sufferer (Acute/Prolonged/Temporary)
- Have suffered Diarrhoea at least once in the past 3 months.
- Must have done self-medication to cure Diarrhoea.
- English speaking (for Malay/Chinese) / Cantonese speaking (for Chinese who can read in Chinese)

Note:
- This is not a sale & marketing position and you are not required to sell or promote any products or services.
- Short-listed candidates will receive confirmation via phone call and sms.
- Your personal data will not be used for any other reasons apart for the survey.

Venue:
- At your home with stable WIFI

Date/Time/Incentive:
- First session (12 April 2020, Sunday): Online survey, RM80 (will bank in into your account within 5 - 7 working days)
- Second session (15 April 2020, Wednesday): Group online survey, additional RM100 (will bank in into your account within 5 - 7 working days), only applicable to those who are selected from first session
- Time to be advised

1. Looking for respondents who can commit for both sessions.

2. The process of online survey:

Step 1: You will receive screening call(s) if you meet the quota balance (i.e.: marital status, age, gender, occupation, monthly household income, monthly personal income, education level, etc) and the quota is still available. The screening call(s) are to make sure that you are qualified to participate in the survey.

Step 2: If you are being selected, you will receive an invitation calls & sms.

Step 3: You will receive a link a day before the online survey via email.

Step 4: You will need to complete the first session of the online survey (RM80, 12 April 2020, Sunday) and we will inform you if you are selected for the second session of the online survey (RM100, 15 April 2020, Wednesday)

Step 5: We will process the payment and you shall receive the payment within 5 working days via bank in.
